The Primary Care Pharmacist is responsible for serving as the medication use expert in the Community Physicians Division of Froedtert & The Medical College of Wisconsin primary care clinics. This position will optimize medication use and patient health outcomes through utilization of collaborative practice agreements for various health conditions, educate patients on medication use, assist with medication procurement/adherence and serving as drug information resource for care team members.

This Primacy Care Pharmacist will be responsible for supporting the Community Physicians Division of Froedtert & The Medical College of Wisconsin primary care clinic locations in the Sheboygan and Port Washington areas of Wisconsin. It is an expectation of the position to work onsite in the clinics. 

The Ambulatory Medical Home Pharmacist is responsible for providing comprehensive pharmaceutical care to patients in the primary care clinics.
The Pharmacist evaluates, identifies and solves medication related problems and acts as a liaison between patients and patients' families and other health care team members regarding medication issues.
Collaborates with physicians, nurses, and other healthcare professionals to ensure the delivery of safe, effective and economic drug treatment.
Other duties as assigned.

Minimum of two years of experience in the primary care or ambulatory internal medicine setting is required.
Bachelor's or Pharm.D. degree received from an ACPE accredited school of pharmacy is required.
State of Wisconsin pharmacist license (or eligible for) is required.

Other information:
PGY1 residency, PGY2 ambulatory care, primary care or internal medicine residency or equivalent experience is preferred.
Demonstrates effective use of computers and applications (e.g. MS-Word, Excel, Outlook) as appropriate to practice and communicate effectively.
